Introduction to Daybreak HCS

Daybreak was formed with the commitment to provide supports and services for people with disabilities. Our guiding values are the foundation of Daybreak: a foundation built to foster an environment where all are encouraged to make and achieve their own personal dreams. Services are tailored through Person-Directed planning to enhance the personal strengths of each individual, enabling each to live and work successfully in the community they choose.

Our goal at Daybreak is to ensure each person supported through Daybreak services have the opportunities they choose and cherish. Daybreak’s focus is on supporting people in their quest for personal growth so that each can experience a Dawn of New Opportunities.

History of Daybreak HCS

Daybreak Community Services, Inc. was founded in 1997 to provide a wide array of Home and Community-based (HCS Medicaid-funded waiver) services to individuals with developmental disabilities. Since that time Daybreak has experienced rapid growth. At the end of the first year of operation, 48 individuals received HCS services from Daybreak. Eight Daybreak group homes designed to serve 3 people each were opened that year in Fort Worth and Lubbock, Texas.

Daybreak currently operates over 40 group homes and also provides services in individual's own homes, foster care settings and day habilitation sites. Services provided to over 320 people throughout Texas are currently funded through the HCS, Texas Home Living, and Deaf/Blind Multi-disability Medicaid Waivers, and through private pay and third-party contract payers.

The management team has an average of over twenty years experience each and combined 175 years + experience in developing and managing programs for people with disabilities.

Daybreak has a long-term commitment to provide innovative supports and community services to people with disabilities. Daybreak's solid foundation enables the provision of environments in which each person may make and achieve goals, while experiencing community life at its fullest.

Leadership

Jeanne Page, CEO
Jeanne Page has been active in the field of Developmental Disabilities for 30 years. She holds a Bachelor of Arts from Texas Tech University and a M.Ed. from the University of North Texas. Her focus has been on the development and management of community-based services for individuals with developmental disabilities in both public and private sector settings. In 1997, she was a principal in the development of Daybreak Community Services, Inc. She has been involved with numerous state committees and work groups over the last ten years. She is a past President of the Texas Association on Mental Retardation and currently serves as a board member of the Private Providers Association of Texas.

Mason Morgan, Chief Financial Officer
Mason Morgan has over 10 years of financial management experience in the health care industry. He holds a Bachelors of Business Studies and a Masters of Business Administration from Dallas Baptist University. Prior to coming to Daybreak, he was a Financial Analyst for Outreach Health Services, Inc. and Chief Financial Officer of Omnicare Associates, Inc. He currently serves as Treasurer for the Texas Association on Mental Retardation.

Susan Hammond, Director of Human Resources
Susan Hammond has 15 years experience in Human Resource Management/ Administration and Payroll for retail and service organizations including 10 years in management positions. She has extensive knowledge of federal, state and local employment laws and has served as a representative for retail companies in state and federal court. She is also author of numerous policies/procedures and training guides for retail and service companies which created and/or enhanced a strong administrative foundation for the front-line team members. Her background and continuing education enables her to function under pressure, demonstrate discretion, integrity and fair-mindedness and to effectively communicate with all levels of the organization.

Rick Hampton, Director of Risk Management
Rick Hampton has over 20 years of experience serving persons with developmental disabilities. He began as a direct care staff member at Denton State School. He was later employed by Fort Worth State School serving in several positions including Human Resources Assistant, Litigation Coordinator, Abuse/Neglect Investigator, Director of Client Records, Director of Management and Program Evaluation Services, Safety Coordinator, and Director of Risk Management. Rick also attended basic and advanced serious incident investigation training from Labor Relations Alternatives of Albany, NY.

Kirk Coats, Regional Director
Kirk Coats joined Daybreak in 1997 and has over 20 of experience in the field of Mental Retardation.  He holds a Bachelors degree in Business Management from Texas Tech University.  Prior to coming to Daybreak he worked at the Lubbock State School for 12 years.  He also serves on PPAT legislative Committee and is a member of TAMR.

Debbie Youngblood, Regional Director
Debbie Youngblood has 20 years of experience working in the field of Mental Retardation. She holds a Bachelor of Business Administration from Texas Wesleyan University. Prior to coming to Daybreak, she was a Program Manager at MHMR of Tarrant County working for the Mental Retardation Local Authority Program and before that, Director of Continuous Quality Improvement at the Fort Worth State School. She is currently a member of the Texas Association of Mental Retardation and a Volunteer and Coach for Texas Special Olympics.

Krista Little, Assistant Regional Director, Dallas-Ft.Worth
Krista Little became a part of the Daybreak team in 2003 with several years of experience in working with people with disabilities.  Krista holds a Bachelor of Arts degree in Behavioral Science.  Prior to coming to Daybreak, Krista served as a Program Specialist with MHMR of Tarrant County.

Cindy Davis, Assistant Regional Director, Lubbock
Cindy began working at Daybreak in 1997. Cindy attended Texas Lutheran College and worked 19 years at Lubbock State School. Through the Habilitation Therapies Department, Cindy specialized in working with people with hearing and visual deficits. Cindy received training at the Helen Keller National Training Center and provides training to Daybreak Staff who serve individuals through the Deaf-Blind Waiver.

Connie Johnson, Assistant Regional Director, San Angelo/Abilene
Connie Johnson joined Daybreak in 2000.  She has over 25 years experience working with people with disabilities.  She started working at San Angelo State School in 1978 in the Recreational Department and was very involved with Special Olympics.  After graduating from Angelo State University with a Bachelor of Science degree in Education she worked with special needs children in the public school before returning to San Angelo State School to serve as a Qualified Mental Retardation Professional (QMRP) and home supervisor. 

 

Services Available

Daybreak is contracted by the Texas Department of Aging and Disability Services to provide Home and Community-based services, Texas Home Living Services, and Deaf-Blind/Multiple Disabilities services. Private pay and other third-party payor services are also available.

CASE MANAGEMENT: A Case manager is available to assist you with service plan development and plan implementation. The case manager meets with you and your family to change the service plan based on changes in your needs and desires, and monitors the services in the plan to make sure it is working to achieve the desired outcomes.

RESIDENTIAL SERVICES:
Supported home living (SHL)
A service in your own home or family home. Services can be provided as you choose or your family chooses to assist in taking you places, helping provide personal care and with learning new things, and helping with medical issues.
Foster Care (FC)
Service where you live with a family or individual who will provide the assistance and support you need.
Supervised Living (SL)
Service provided in a group home with 2 or 3 other people. However, the staff can be allowed to sleep during the night.
Residential Service Support (RSS)
Service provided in in a group home with 3 other people where awake assistance is provided 24 hours a day.

Supported Employment (HL)
Provided after the Texas Rehabilitation Commission has helped you get a job. This service will provide staff to help you keep your job.

Minor Home Modifications (HL)
Modifications could include installing rails in a bathroom, widening a door for a wheelchair etc. The modification must be approved by a therapist and/or doctor and the Department of Aging and Disability Services before the work is done.

Respite (HL)
Provided if receiving supported home living services and you or your family needs someone to stay with you. A staff member can stay with you at your home or you can stay at another home provided through Daybreak.
